How to Request a Delivery Instruction Slip (DIS)

If you selected Option 1 for Delivery Instruction Slips (DIS) during account opening, the DIS book was sent to your registered address. If you selected Option 2 (No), you will need to request a DIS manually.

Online Request for DIS:

  1. Access the Re-KYC Module: Click here to go to the Re-KYC module.

  2. Enter Your Details: Provide your UCC (Unique Client Code) and Date of Birth, then submit. An OTP will be sent to your registered email ID and mobile number.

  3. Enter the OTP: After entering the OTP, you will be directed to the Re-KYC dashboard.

  4. Request DIS: From the side menu, select 'DIS Req' to place your request for a DIS.

Offline Request for DIS:

Alternatively, you can download, print, and sign the DIS Request Form and send it to the following corporate address:
